1.2       Statement of the Problem

It is significant to note that Lifting and hoisting operations are among the major causes of reoccurring fatalities and serious incidents in global industry activities, with no exception to selected companies in Onne High profile of recent accident is a major problem in the lifting operations at ONNE.

Its indeed worrisome to note that, in spite of the numerous regulations such as the lifting operation, lifting regulation LOLER 1998,Provision and use of work equipment regulation 1998(PUWER), operating standards and guidelines Factory Act 2004, etc as applicable to lifting operations, preventable incident continue to occur.

Problems associated with lifting operations may include:Operational ground situation and environmental conditions. Poor soil condition constituting uneven distribution of load Another problem is the randomly aligned overhead powerlines. Maintaining the required distance of minimum of six(6) meters fromoverhead power lines for high voltages posea problem.

In climate weather conditions of rains and water create wet and slippery ground, loose soil leading toconditions that threatens and decrease the safe working load, for wind speeds above 50 kmph.

The use of poor subcontractor for lifting operations often pose some problems as regards the conditions of the crane, untested gears as statutorily required.

Lack of proper training and competency of the lifting team without evident confirmation of their knowledge and experience of the crane operator, rigger, bankman or signaler etc pose serious problem to safe system of lifting operations.

The ineffective use of offshore wind turbines (OWTs) resulting into rapid decrease of costs of installation and grid connection to energy sources, has generated some problems in offshore wind turbine (OWT) that lower crane capabilities, hence lifting operations are conducted per pragmatic experiences and interim of weather forecast with increase for safety hazards.

Obvious lack of mandatory lifting plan, supported by operation specific risk assessment for all critical lifts do not only pose problems but often result into fatality and severe injuries.

For instance, one of the major causes of fatalities during construction involves unsafe lifting operations that utilize cranes, and this accounts to about 12% of all the construction worker fatalities. The recent rash of crane accidents internationally hasheightened the public’s awareness of the potential hazards posed by lifting operations and this necessitates the present study, focusing on selected companies at Onne.
